HEALTH HOME PRIMARY CARE LIAISON - PRIMARY CARE INTEGRATION WITH BEHAVIORAL HEALTH
This position is responsible for providing direct clinical services to patients within the scope of services allowed under the DC Board of Nursing and in accordance with the rules and regulations of the Psychiatric Committee and Medstar Washington Hospital Center (MWHC) Medical Staff By-Laws.
This includes performing psychiatric evaluations on patients, in collaboration with a supervising psychiatrist and a treatment team, prescribing medications, providing clinical assessments and diagnosis in accordance with DC law and MWHC Medical Staff By-Laws and other duties as assigned.
This position is responsible for applying evidenced-based treatment for the primary care of individuals with chronic medical conditions and complex co-morbidities. Treatment of patients will primarily be provided in a comprehensive behavioral healthcare setting. The Advanced Practice Registered Nurse (APRN) will prescribe medication in accordance with generally accepted practices; formulate a goal directed plan of care, in conjunction with the multidisciplinary treatment team, using assessment data from nursing, social work and other activities, ensure care consistent with physical and behavioral health standards; establish priorities of patient care based on available resources of time, personnel, equipment and supplies; monitor Health Team for each consumer, such as medical history, physical examination, activities assessments, social worker’s assessment and community referral information, recommend and assist in the implementation of adjustments to enhance health and wellness and work with Clinical Manager, Nurse Care Manager and other behavioral health staff to monitor utilization and outcomes for consumer populations.
Must be an Advanced Practice Registered Nurse (APRN) Nurse Practitioner with experience and expertise in applying evidence based guidelines to provide primary care for individuals with chronic conditions and complex co-morbidities. Prefer minimum of 3 years practice experience as an APRN.
