SUMMARY PERFORMANCE DESCRIPTION:

Under the direction of the MD, RN or licensed Social Worker, the discharge planner facilitates patient's aftercare needs, communicates directly with providers about the acute and post-acute appointments and order needs; and independently schedules follow-up physician, labs, home health, nursing home placements, procedural, and financial business office appointments.  Additional duties include obtaining insurance authorizations, arranging transportation, completing forms, faxing, filing scanning, and documentation.

SPECIAL KNOWLEDGE AND SKILLS:

Knowledge of or experience in mental health or chemical dependency fields; ability to work under pressure and meet deadlines; ability to adapt to workload demands and set priorities; positive, professional presentation; ability to build rapport quickly and develop working relationships; ability to work cooperatively with the program staff; good communication and organizational skills. Requires access to transportation.

EDUCATION:

Bachelorette degree in Social Work preferred. High School Diploma required.

PRIOR EXPERIENCE:

Minimum of 3-5 years of experience in the behavioral health field. Knowledge of mental health resources in the community and how to access; computer and electronic medical record (EMR) technical experience.
