Signet Health is actively interviewing Licensed Clinical Social Workers for a full-time position with our program at Franciscan Health in Michigan City, Indiana.

The Social Worker collaborates with individuals, families, and groups to address psychosocial challenges through a process of assessment, diagnosis, treatment, and evaluation. As part of a multidisciplinary care team, this position works directly with patients and their caregivers to resolve personal crises and support a therapeutic milieu to cope with chronic, acute, or terminal illnesses. The Social Worker is also responsible for overseeing the general case management and care coordination of assigned patients.

 

Essential Functions:

  • Assess the psychological status of patients related to the patient’s illness and environment.
  • Carry out social evaluations and interventions based on evaluation findings.
  • Provide information to patients or families and care givers and community agencies.
  • Provide patient and family-focused therapeutic crisis counseling.
  • Continuously update case manager.
  • Maintain clinical records on all patients referred to social work.

 

Requirements/Qualifications

  • Licensed Social Worker (LSW) - Indiana Licensing Board
  • Licensed Clinical Social Worker (LCSW) - Indiana Licensing Board
  • 2 years Acute Care Hospital experience (preferred)
  • 2 years Mental Health or related Community Healthcare (preferred)
