Our services include brief therapy, group therapy, addiction related services, medication management, Intensive Outpatient Treatment Program (IOP), Partial Hospitalization Program (PHP), and integrated Co-Occurring treatment for adults.

 

We have a Full Time position available for a Care Coordinator opening that offers rich and unique learning opportunities.  Typical schedule will be M-F from 8:00 am-5 pm.

Overview

Position Overview:

The Ambulatory Behavioral Health Care Coordinator is a clinical support role designed to assist in the coordination and delivery of care across the full continuum of behavioral health services—general outpatient, Partial Hospitalization (PHP), Intensive Outpatient (IOP), and treatment addictions. This position supports daily operations, assists in managing complex/high acuity patient needs, and helps ensure smooth transitions of care and compliance with treatment protocols. The coordinator will also support patient registration, address general patient needs, and assist in crisis response as requested by service line leadership. This is not a licensed clinician role but is clinical in nature and reports directly to the Clinical Director.

Requirements/Qualifications

Key Responsibilities:

General Outpatient and Clinical Support:

  • Assist with patient registration, coordination of appointments, and general clinic flow as needed.
  • Support with transitions between outpatient levels of care, including PHP, IOP, group therapy, individual therapy, and psychiatric medication management.
  • Participate in interdisciplinary treatment planning, including input on care plans and coordination of services.
  • Serve as a liaison between internal teams (e.g., medical, therapy, case management) and external partners (e.g., community providers, CSAs) to ensure continuity of care.
  • Provide limited case management for a small caseload of high-need or short-term patients, including safety checks, appointment follow-up, and medication refill coordination (caseload monitored by Clinical Director).
  • Support crisis and urgent patient needs through in-person or telephone follow-up, including coordination with clinical and administrative teams.
  • Assist in resolving patient concerns or complaints in collaboration with leadership.

PHP/IOP Program Coordination:

  • Attend PHP/IOP team huddles and support team communication.
  • Make engagement calls and follow-up contacts with patients and providers (e.g., no-shows, referral issues, complaints).
  • Assist with scheduling PHP/IOP patients for medication management appointments
  • Support with crisis intervention tasks, including welfare checks and urgent outreach.
  • Provide short-term resource navigation (e.g., food, transportation, housing, emergency needs).
  • Collaborate with the admin team to ensure intake documents are completed and scanned properly
  • Conduct weekly check-ins with PHP/IOP leadership and assigned patients as needed.
  • Monitor completion of clinical screeners
  • Maintain PHP/IOP room supplies, ensure environmental safety and compliance.
  • Document all interactions in MedConnect via Communication Notes.
  • Support treatment plan transitions between levels of care.

Administrative & Operational Duties:

  • Assist with insurance coordination, including supporting patients in obtaining Emergency Medicaid.
  • Maintain accurate patient data, referral information, and follow-up tracking.
  • Coordinate with medical providers (MD, RN, MA) and support general psychiatric clinic flow.
  • Support documentation compliance, and other operational needs across programs.
  • Other duties as assigned by the Clinical Director or service line leadership.

Hospital/Program Description

Qualifications:

  • Minimum 2 years of experience working with psychiatric or behavioral health patients.
  • Strong interpersonal skills and ability to collaborate with interdisciplinary teams.
  • Familiarity with behavioral health systems, community resources (DC, MD, VA), and levels of care.
  • Proficiency in Microsoft Office and EMR systems (e.g., MedConnect).
  • Organized, flexible, and able to prioritize in a fast-paced clinical environment.
  • Excellent written and verbal communication.
  • Bachelor’s degree required; Master of Social Work (MSW) or related field strongly preferred.
  • Licensure (e.g., LGSW/LGPC) not required but preferred.
  • Experience with adult behavioral health and substance use treatment is preferred.
  • SAFE (Safety Assurance for Escalating Behaviors) training must be completed and maintained.

Compensation Information:

 

Base Salary: $64,000 Annually
